The hip joint connects the pelvis to the leg bone or femur and is one of the largest joints in the body after the knee. It is a ball-and-socket joint allowing for a wide range of motion in all directions.
The hip joint is stabilised by several ligaments which support the joint and prevent excessive movement. Several muscle groups connect to the hip joint to allow motion of the legs, including the gluteal muscles, hamstrings, quadriceps, hip adductors and hip flexors.
There are many possible causes of hip pain. If your hip pain occurred suddenly, for example by an awkward twist of the leg or when lifting something heavy, you may have suffered a sprain or strain to the muscles, ligaments or tendons around the hip. Try to rest the affected area as much as you can, and make an appointment for treatment. Some people also feel that icing the affected area can help in the first 24-48 hours. If you do use ice, take care to do so safely.
If you are very active or play sports such as football which are likely to put strain on the hip joint and you feel your hip pain has come on slowly and occurs intermittently, you may be suffering from a repetitive injury. This could be a tendinitis – inflammation of the tendons in the hip – or bursitis, where the fluid-filled sacs or bursae become inflamed, often due to overuse.
There are many other possible causes of hip pain. Certain hip pains can be caused by gynaecological or pelvic floor issues, or by problems or misalignment elsewhere in the body, such as foot pain.
Arthritis is a common cause of hip pain, especially in older people. There are various types of arthritis which may be causing your pain. In the case of osteoarthritis you may feel the pain is worse with movement and experience stiffness after walking. Osteoarthritis is a degenerative disease which occurs when the cartilage which cushions the joints wears down over time, and affects many people as they get older.
Rheumatoid and psoriatic arthritis can also affect the hip joint. These are autoimmune diseases which lead to joint inflammation. Conversely to osteoarthritis, you may feel pain from lack of activity – for example when you first wake up in the morning.

Physical therapy is an essential part of a treatment programme following hip replacement surgery. This will include treatment to increase the strength and mobility of the new joint, treatment to help you use the new joint such as gait training, and exercises to do yourself to build the functionality of the new joint.
